A woman and a young daughter are brought into Holby after a terrible car accident in which a large steel rod has pierced the daughter's liver and abdomen. Ric asks Abra to attend to the daughter.
The Director of Surgery job is the main topic of conversation at Holby as Jayne informs Ric and Connie that the position is now open to other candidates. Connie isn't impressed but confidently tells Ric that this won 't make any difference to her application, it will just make the process longer and more difficult before Jayne gives her the position.
Jayne is having a stressful day as the Home Office is visiting Holby to look into the situation with the Tans, the North Korean couple expecting conjoined twins. Jayne asks Ric to oversee their visit she feels that he is the best person to speak to now that the patient is under his care.
Connie is under pressure to make sure Jayne knows that she is the best surgeon at Holby and she performs a new procedure to the UK, "Mitral Value Regurgitation". The patient is Mrs Forrester, a "massive " businesswoman who is desperate to be the centre of attention. Mrs Forrester's ego is fuelled as Connie brings in a team of young medical students to oversee her treatment and the procedure is going to be streamed live on the Internet.
Connie bumps into Michael who informs her that he is also going up for the Director of Surgery position at Jayne's request.
Connie is confused as to why Jayne would do this until Elliot tells her that Michael has written a paper that is receiving hundreds of hits on the internet and that he is very knowledgeable on the subject.
The home office representative, Veronica Peterson arrives at Holby City to look into the Tan's situation. She is met by Mark who takes her to the Tans but she cannot ask the couple any questions as there is no interpreter and so she has to wait to see Ric.
After taking a blood sample from Liam Harris, the patient who died whist under Linden's treatment, Maddy is desperate for the results and hunts around for them. She hadn't put her name on the sample but used Sam's name instead so no one would notice her "snooping ". As she finds the results, Sam grabs the letter from her asking her what she was doing. Maddy lies and turns the conversation around by asking Sam if he will stand in for her in theatre, Sam looks confused as he knows Maddy loves being in theatre. Maddy makes up another lie about filling in papers so Sam agrees - but he's confused as to why Maddy would rather be filling in documents than be in theatre.
Meanwhile, the condition of the young car accident victim deteriorates as Abra has difficulties concentrating in theatre and he seeks out help from Ric and Michael. Michael steps in to help Abra who watches rather than actually playing a part in the operation. Ric also steps in to help as the girl's condition quickly worsens and Michael and Ric have disagreements on how to remove the steel rod from her abdomen.
They both argue about which will be the safest and quickest way. "We're going to have to yank it out, kid " says Michael but Abra is disgusted by the notion that they would even dream of "yanking it out". Ric disagrees and says they should perform a "Midline Laparotomy ". Michael believes it will take too long but agrees that it will be the safest way to save her liver, which the rod has penetrated. They perform the procedure, with Abra looking uncomfortable as he watches over the young girl. A mass of blood erupts from the girl 's body and as Ric asks Abra for suction Abra just stares at the blood, lost for words before returning to the ward.
Sam covers for Maddy in surgery but Jamie is not concentrating, as he is more interested in getting the young nurseâs attention. Jamie fails to give the patient the correct fluids and Sam is forced to intervene. After surgery Sam opens his letters to find Liam Harris 's results. He is annoyed at Maddy for putting the sample in for tests under his name which - if someone had realised it was the blood from the dead body - would have seen him lose his job at Holby. Maddy informs him that she believes it was in fact Jamie's fault for not concentrating in theatre with Linden. Sam realises why Maddy didnât want to go into theatre with him so he could see what Jamie was like.
Sam informs her that the tests came back negative; Maddy looks disappointed, as she believed that the lack of fluids caused Liam Harrisâs death. Sam agrees that something could be wrong after witnessing Jamie's lack of concentration in theatre and tells Maddy that they should do more tests on Liam Harris body fluids. Maddy informs him that she can 't get any more samples from his body so Sam thinks of an alternative plan.
Connie 's big moment seems to slip away as Mrs. Forrester "brags " to Donna about how she and her husband had drunk 20 year old whisky just two nights before. Donna informs a stressed Connie that Mrs Forrester has been drinking and so Connie orders Donna to do more tests on her quickly to find out if she is in fact fit for surgery.
Michael and Ric's efforts to save the young girl fail as she dies from her injuries. The Home Office representative is getting annoyed as she waits for Ric and she asks Abra to tell her about the Tan 's condition. He looks over the chart and informs her that they are fine and that nothing is too serious. Ric finally meets the representative and despite him fighting the Tan 's case she is not convinced after what Abra had just told her but Michael steps in and gives her a second opinion which convinces her of their plight. Ric is annoyed at Abra and asks him to pull himself together and to tell the parents of the young girl that she hasn't made it.
Abra, informs the father of the young girl about her death as the mother is suffering from short-term memory loss. The father breaks down and asks Abra how he can tell his wife, Abra simply tells him "Say what you like" and leaves. The father needs help and he attacks Abra until Michael step in to comfort the father.
Connie gets the results from Mrs. Forrester and finds no traces of alcohol and believes Donna was lying. The operation goes ahead but Connie is upset that Jayne wasn't there to witness her excellent work.
