Annalese comes into work with Michael and is seen looking at his credit card payments as they get out of the car. When she sees him whispering with Donna, she becomes suspicious.
Annalese gets on the phone, trying to glean some information about Michael 's account, but she is denied access. She asks Michael if he remembers their first date, and says that they should go away together, just the two of them. But Michael says he is far too busy. Michael has fun with his patient, Simon Hayder, a young man with Aspergers' syndrome. He challenges Simon's encyclopaedic knowledge of his personal medical condition and implies the doctor knows best.
Jayne Grayson tells Ric that she thinks the zero tolerance campaign is going smoothly and that any concerns the board may have had are waning. Connie and Elliott both decide to miss Ric's planned talk on zero tolerance and when Ric asks Michael about it he says that he's not going to rock the boat for the time being. Connie tells Ric that his six months as director of surgery are finished and as they made a deal that he was going to step down for her, it is now time for him to leave. Ric says he will speak to the board about it but it is clear that he is very reluctant when Connie makes him shake on it.
Joseph tells Linden that he has not had any chance to confess to Faye yet that he slept with Jac as it has been so hectic. When he tries to apologise to Jac about the way he handled everything, she says she thinks he's said enough.
It's Daisha's 30th birthday but she doesn't want to celebrate. However, her friends throw a surprise party in one of the rooms at the hospital and organise drinks for later.
Ric finishes his lecture on the zero tolerance policy but Elliott tells Ric that he thinks they are playing God and that he should talk to people about what they can do, instead of what they can't.
Frankie, the son of Geoffrey Moore, the obese man who was refused treatment, needs a chest drain, but his body fat makes the procedure much more difficult. Connie goes to consult Ric about whether this is another zero tolerance case, hoping for trouble, but Ric says it is an emergency situation so Frankie should be allowed to have it.
Ric takes it upon himself to lecture Frankie about his and his father's unhealthy lifestyle. Frankie understands and doesn't want his father to come to the hospital. He says that his father was better when his mum was still alive as she used to look after them, but Ric feels that he shouldn't try to make excuses.
Jac mentions that she is not drinking and when Faye enquires why she says it is for a detox. Joseph rushes over to find out what they were talking about but when he tries to say something he chickens out.
Connie thinks Ric shouldn't lecture patients just before a procedure and complains to Jayne Grayson as she thinks Frankie is being used as a pawn. She also draws Jayne's attention to the now vacant DOS position, as she believes it is not clear who is in charge anymore.
Annalese sneaks into Michael's office and checks his emails, where she finds a room booked in a five star hotel in Rome . She hears Michael coming and quickly hides evidence of her snooping. When Michael comes in he tells her that he has to go to Rome for a conference, but promises that he will take her there another time.
Rachel offers to babysit for Daisha who is concerned about taking advantage of Mark. Mark's patient sees that he is besotted with Daisha and tells him, but Mark tells him that he has misinterpreted the situation as Daisha is young enough to be his daughter. His patient tells Mark that she likes him as Daisha comes over to ask him for drinks. Maddie discovers that the patient has glaucoma, and not only will she have to stop driving, but her whole life will change, which upsets her.
Connie speaks to Frankie and says that she doesn't have a problem with Dr Griffin but he likes being in charge and doesn't want to give up what he loves most in life. Frankie becomes agitated and Connie has to call Faye in to help.
Before Frankie's operation, Connie asks Jac to find Jayne's PA and ask if she has any emergency meetings. Connie and Ric do Frankie's operation together, there is a bit of a power struggle during it between the two, but in the end they work together. Connie keeps referring to Ric's resignation and won't let it lie. Later, Jac tells Connie that there were no emergency meetings booked for Jayne.
Donna asks Annalese if there is something wrong, and Annalese apologises for being grumpy, but then spots a travel guide to Rome in Donna's locker.
Donna, Michael and Annalese are in theatre together operating on Simon and Annalese is very catty to Donna, asking if women marry men who are like their fathers. Simon goes into cardiac arrest during the operation and Michael realises that Simon's earlier self-prognosis was correct. After the operation Donna is clearly upset and Michael is annoyed. But first he must tell Simon he will be OK and apologise for ever doubting his medical knowledge.
Michael talks to Annalese and asks about her question to Donna in the theatre. He knows that Annalese's father cheated on his wife, left his family and died alone, but says that they will talk about their future at home.
Faye asks Joseph if he thinks Jac is seeing anyone, as her women's intuition is telling her that she is pregnant. Joseph rushes off as quickly as possible, saying that the idea is fantasy.

At her birthday drinks Mark gives Daisha a bracelet, the one she saw before (when?). At first she is reluctant to accept such a nice present, but eventually she does.
