Tom finds the patient for whom he did a personal favour, only to discover he's drunk the whole bottle of wine himself. The patient puts up a fight and causes a commotion - Tom doesn't need this right now.
Annalese is still in her car, when Ric sees her. He says how sorry he is that she's been suspended but it's too much for Annalese. She starts to cry and desperately needs to talk to someone about the state of her marriage. Ric takes her aside and she starts to tell him all about Michael's affairs and his fling with Donna.
The patient Michael had bumped has now become an emergency and he has to operate quickly. He requests that Maddy attends and in theatre fishes to find out what Maddy remembers about the Hewitt operation. But Maddy won't play ball - she knows what Michael is doing and she doesn't want to be compromised - she'll tell everyone the truth.
Tom's patient has re-opened his wound so Tom has to perform the minor procedure alone. At first it goes well and the patient's condition is immediately improved but just as he is finishing off, he goes into crisis. Tom tries to find Maddy to help, but she's off the ward. Ric finds Tom with the patient and can't believe what has happened. Ric salvages the situation, and saves the patient.
The operation with Samson goes well but it's not over yet. Ben is outside the theatre, keeping watch as Connie approaches. Elliot and Jac are at a vital stage in the op. Connie asks Ben to step out of the way but he stands his ground. Inside the theatre, Martha can hear Ben's attempts to persuade Connie to go away. It's clear to Connie that something is afoot and glimpses Elliot in the theatre with the dog on the table. She can't believe what she's seeing, but Martha talks to her explaining that the dog means everything to her father. Can she just turn a blind eye this once. Connie agrees but wants everything sterilising afterwards.
After the operation, Michael seeks out Annalese who finally feels able to talk to him after her breakdown to Ric. She tells Michael what he already knows - that she has been suspended and is due to meet their lawyer.
Annalese meets with Charles, who realises she is the woman who nearly ran him over. An embarrassed Annalese sits as Michael weaves his magic and explains everything. It's an amazing performance but Annalese can't take it. She explains that she killed Hewitt. Michael tries to correct her but Annalese has had enough of lying and wants the lawyer to know she is guilty. Charles leaves them as they clearly have issues to sort out.
With no more guilty secrets, Annalese is starting to feel unburdened. For the first time Annalese starts to open up to Michael - explaining how betrayed she feels and how she has now accepted that she needs a new life away from him. Michael tells her to think about her future - why is she punishing herself over this - it's his fault, not hers?
Annalese is about to leave Holby when Ric catches her and reminds her that she is a brilliant anaesthetist - she's too good to throw it all away just because she is worried about her future with Michael. He is sure that the whole thing will blow over and hopes to be working with her in the future.
Elliot manages to stabilise Samson and finish the operation. Elliot carries his dog to the office and waits for him to come round from the anaesthetic. Samson opens his eyes. Elliot is overjoyed, Jac smiles and Elliot thanks her - he doesn't know how he can ever repay her. Martha comes into the office to say that Connie is on her way. Martha and Jac move the dog next door just as Connie opens the door, to see the other door to the office being closed. Elliot is sitting at his desk, he smiles at Connie, she drops a present on Elliot's desk- a dog chew. Elliot smiles - he knows Connie cares.
It hasn't been Tom's day, but it seems to be over. Tom and Maddy end the day drinking in the bar. Ric notices the two of them and he looks over to see the two laughing and joking.
